Enrol your child

Your child’s first day starts here.

Our enrolment form gathers everything we need to care for your child: their details, your family’s contacts, health information and authorisations. It takes a little while, so here’s what to have on hand.

Have these ready

For documents, a clear photo or scan from your phone is fine.

Details

  • Centrelink Reference Numbers (CRN) for your child and for you
  • Your child’s Medicare card number and expiry date
  • Your child’s doctor and dentist, if they have one
  • Names and phone numbers for two emergency contacts

Documents

  • Your child’s birth certificate
  • Your child’s immunisation record
  • A recent photo of your child
  • Photo ID for each emergency contact

If they apply, also have your child’s medical action plans (anaphylaxis, allergy or asthma), any court or parenting orders, a Health Care Card, or an NDIS plan.
